According to American nurses association state nurse sensitive indicators reflect the structure, process and outcome of nursing.
Falls : Fallbis one of the most sensitive indicator which adversely affect the quality of health care. Fall is unplanned with or without injury due ti slipping on floor, fainting or fall from bed. The result of fall may causes minor, moderate, major injury or even death may occur. It is very essential to maintain a protocol for assessing the fall, risk factor and prevention.
Nursing action :
- Fall risk assessment
- Clean and dry the room.
- Use side rails up
- Use non slip mats and treads
- Improve lighting
Restraint
It may be used to prevent falling during procedure and control ir prevent harmful behavior. Physical restraint is that movement restricted. Chemical restraint is restrictions of behavior by medication.
Nursing action
- Avoid the risk if injury due to restraint. Restraint should not be too tight
- Should not oversedated.
VAP(ventilator assisted pneumonia )
It is caused by infection of patient who is on mechanical ventilator which leads to illnesses and death. It is a nosocomial infection.
Nursing measures :
- Follow strict aseptic technic while handling the patient on ventilator
- Antibiotics should be given as per doctor order.
CLABSI(Central line assisted blood stram infection )
It is occur due to improper handling aseptic technique and also the infection occur debilitated patient such as elder patient, immunocompromised patient .It is due to negligence of staff. It can be prevented by follow strict aseptic technique. Use chlorehexidine for skin preparation.
