Healthy people 2020 overarching goal are achieving health equity, longer lives free of preventable diseases, eliminate disparities, create social and physical environments that promote good health for all, attain high-quality etc. Here we will pick the topic, longer lives free of preventable diseases.

Epidemiological and statistic data to be collected in order to plan a community outreach hospital-based program varies depending on the goals. Here in our case, need to collect the data of preventable diseases that are affected by the targeted community. Evaluating the trends in the last few years data will give you the overall picture of the community. Next step is to discuss the methods to control those situations or diseases. We need to consider age groups, gender and physical environments while collecting statistic data.

Out of the different epidemiological tools such as internet research, telephonic and face-to-face interviews, observations, surveys, we will choose face-to-face interviews as the most effective tool. It will stand out from other methods by ensuring the authenticity of data collected. Finally, with the data collected, we will be able to compare the risk factors, attack rates and response rates and can lead the outreach program in the community to great success.
