Fever or rise in temperature is a common sign when a person gets an infection. Kernig's sign occurs in meningitis. Kernig's sign is positive if a person gets pain during extension of his leg when he is lying flat with knees and hips flexed.

Brudzinski's sign is positive in meningitis patients. When a person's neck is flexed when he is in lying down position, his knees and hips will be flexed because of neck stiffness.

photophobia is sensitivity to light occurs in most of the neurological conditions which is also common in meningitis especially bacterial meningitis.

Petechial rash is a type of rash which occurs in advanced meningitis. It is also called purpuric rash which results from bleeding beneath the skin.

A) Meningitis

Meningitis is the inflammation of the membranes surrounding the brain and spinal cord. It is caused by virus, bacteraemia or rarely by fungal infections. The common symptoms are severe headache, stiffness of neck, fever and vomiting.

Bacterial meningitis is life threatening and it is caused by bacteria such as streptococcus pneumoniae, Niesseria Meningitidis.  

Viral meningitis is mild and often goes off easily. Fungal infection is uncommon and it is caused because of immune deficiencies like HIV infection.

Symptoms of meningitis are severe headache, fever, stiffness of neck, vomiting and if not diagnosed and treated early will lead to severe meningitis.  

Signs of meningitis include positive kernig's sign, Brudzinski's sign, photophobia, petechial rash and mental status changes like confusion, loss of alertness, changes in perception and disorientation.

Hence the correct answer is option A.

B) Optic neuritis

Optic neuritis is the inflammation which damages the optic nerve. The symptoms are the person cannot see properly, he cannot see the colors and his eyes will hurt when he moves his eyes.  

Hence option B is incorrect

C) Trigeminal neuralgia is a condition in which the patient gets severe pain in his face. The pain is triggered by speaking and chewing.

Hence option C is incorrect.

D) Migraine headache is caused by some genetic, hormonal and environmental factors. It is usually one sided headache and the person will be sensitive to light.
